Mesothelioma Cancer

Making a correct mesothelioma diagnosis is particularly difficult for doctors because the disease often presents with symptoms that mimic other common ailments. There is no known cure for mesothelioma, but treatments such as surgery and chemotherapy have helped to improve the typical mesothelioma prognosis.

Pleural mesothelioma (affecting the lung’s protective lining in the chest cavity) represents about three quarters of all mesothelioma incidence. Peritoneal mesothelioma which affects the abdominal cavity and pericardial mesothelioma, which affects the cardiac cavity, comprise the remainder. Testicular mesothelioma is extremely rare and is typically presents with metastases of the peritoneal variety. There are three recognized mesothelioma cell-types. Between 50 and 70% of all mesotheliomas are of the epithelial variety. While prognosis is generally poor, it is considered less aggressive than sarcomatoid mesothelioma and biphasic mesothelioma, which comprise the remainder of cell type diagnoses. The cavities within the body encompassing the chest, abdomen, and heart are surround by a membrane of cells known as the mesothelium. Mesothelial cells assist in general organ functions. The mesothelium is particularly important to organs that are commonly in motion, such as expansion or contraction of the lungs, stomach, or heart. Lubrication from the mesothelial cells allows free range of motion within the body. The mesothelium of the chest, abdomen, and cardiac cavity are called the pleura, the peritoneum, and the pericardium, respectively. Each of these groupings of mesothelial cells are extremely critical to the functions of the body structures which they encompass.


Malignancies (cancerous tumors) occurring within the mesothelial membranes are known as malignant mesothelioma, or simply mesothelioma. Benign tumors of the mesothelium are known to occur, but are much rarer than the more common malignant cancer.

While tumors of the mesothelium were first recognized in the late 18th century, it was not until the middle of the 20th century that this particular cancer was studied and examined with more detail. It was at this time where suspicions of the cancer’s causal relationship with asbestos exposure became more substantiated. A joint research venture through the Department of Thoracic Surgery at the University of the Witswater and/Johannesburg General Hospital in South Africa provided the most compelling evidence of the nexus between asbestos exposure and the development of pleural mesothelioma.

Incidence of mesothelioma is still quite rare, with only 2,500-3000 diagnoses in the United States each year. There was a spike in reported diagnoses between 1970 and 1984, which has been attributed to the latency period between diagnosis and the height of industrial exposures, which occurred roughly 40-60 years prior to this time. Exposure was common in nearly all industries but was particularly common in the WWII-era military industrial cycle, including Navy Shipyards.


Although this cancer is much more common in men over the age of 60 (largely attributed to the industrial exposures within male-dominated industries), mesothelioma in women and children has been described as well. Mesothelioma causes for diagnosis in women and children are mainly attributed to secondary exposure to asbestos, as it was not uncommon for men to bring asbestos back into the home on their body or clothing if proper cleaning facilities were not available on site.

Mesothelioma is diagnosed through a comprehensive combination of biopsy and imaging scans.

Mesothelioma can be a difficult malignancy to diagnose because the symptoms and pathology of the disease closely resemble other respiratory conditions. For this reason, misdiagnosis is not uncommon in mesothelioma patients. Symptoms of mesothelioma include chest pain, chronic cough, effusions of the chest and abdomen, and the presence of blood in lung fluid.

Diagnostic surgeries, including a biopsy, will typically be required to determine the type of malignant cells that are present in the body. Typically a body imaging scan, including a magnetic resonance image (MRI) or computer topography (CT scan) will be required to determine the extent and location of the disease.

Aggressive surgeries such as extrapleural pneumonectomy can extend survival rates far beyond previously-thought timeframes. Diagnostic and palliative surgeries such as pleurocentesis and pleurodesis are also common in patients of malignant mesothelioma cancer.

Alternative therapies have also been used effectively by many mesothelioma patients to assist in managing symptoms of the disease and conventional treatments. These treatments are mainly preferential but can be extremely valuable to many patients.

Mesothelioma Treatment

If there’s one thing we know about mesothelioma treatment, it’s this: we’re still being challenged to arrive at efficacy.

Despite years of research since the disease was first identified, it’s still difficult to identify the best approach to treating the disease, says David Rice, MD, a cardiothoracic surgeon and nationally known mesothelioma expert who practices at the University of Texas MD Anderson Cancer Center in Houston, TX.

The very rareness of the cancer—only about 3,000 people a year are diagnosed in the United States—makes it difficult to run the kind of research studies needed to compare treatments and determine the ideal therapy at each stage of the disease. “There isn’t a lot of evidence-based science in this disease,” Dr. Rice admits. So when his patients ask him what the best treatment is for the disease, he tells them what we tell you in this section, adding that “we don’t have a reliable cure for this disease.”

Thus, a major goal of treatment is to reduce pain and suffering and prolong a patient’s life as long as possible while providing them with the highest quality of life possible.

Choosing the right mesothelioma doctor is an important first step in planning for treatment.

There are a number of mesothelioma experts, like Dr. Rice, practicing in specialized clinics throughout the country. Each of these cancer specialists has an acute knowledge of the behavior and pathology of malignant mesothelioma and its treatment. The most important consideration in mesothelioma treatment is the cancer stage and type, said Dr. Rice. Treatment decisions also depend on whether the cancer is localized to the chest or has spread to the chest wall, diaphragm, or lymph nodes, your age and overall health, and the center where you’re being treated. Conventional treatments for mesothelioma involve surgery, chemotherapy, and radiation therapy.

As with most solid tumors, doctors turn to surgery, radiation and chemotherapy to manage mesothelioma. When exploring the various treatment options available with your doctor it is important to be informed about the risk and benefits of each one before making a final decision.

Surgery

Only about 1 in 5 patients with metastatic pleural mesothelioma undergo surgery. There are two main surgeries: pleurectomy/decortication, in which the surgeon tries to remove as much of the tumor from around the lung as possible, and the more radical extrapleural pneumonectomy, in which the lung itself is removed.

There is a lot of debate as to which is “best,” said Dr. Rice, although studies find that most long-term survivors have had surgery. He personally believes that pneumonectomy is best for tumor control if followed with radiation. Studies find it prevents tumor recurrence in the chest in 80 to 85 percent of patients who have the surgery.

However, Dr. Rice noted, it is a long, intensive operation with a 55 percent complication rate and a 3 percent risk of death, higher in some institutions. “So you only want to perform that surgery if the patient has a reasonably good prognosis,” said Dr. Rice. Translation: it doesn’t appear that the cancer has spread outside the chest.

Patients best suited for pneumonectomy are younger, with the epithelial form of the disease, no obvious lymph gland involvement, and are otherwise healthy enough to withstand the rigor of the procedure. Dr. Rice actually waits until he has the patient’s chest open in the operating room and biopsies the lymph nodes before deciding which procedure to perform.

Pleurectomy/decortication has a higher failure rate, with the tumor recurring in the chest cavity 50 to 80 percent of the time. However, that rate may change with improved radiotherapy techniques, Dr. Rice said. The reason for the high recurrence is that it’s impossible to completely remove the tumor without removing the lung.

However, he noted, there is no difference in survival rates between the two surgeries. Part of the reason is that the cancer has often spread to other parts of the body by the time it is diagnosed even if it appears to be confined to the chest.

Chemotherapy

Chemotherapy, also known as systemic therapy, uses oral or infusion-based medications to kill cancer cells throughout your body. Chemotherapy is used both before and after surgery, as well as in people who can’t handle surgery. It is also used in the palliative setting to reduce pain and improve quality of life.

The most commonly used chemotherapy drugs for mesothelioma are cisplatin combined with pemetrexed (Alimta) or raltitrexed (Tomudex). Other combinations include gemcitabine, carboplatin or oxaliplatin.

If you can’t manage combination therapy, your doctor may start on just one drug. Sometimes, your doctor may infuse the medication directly into your chest cavity, a procedure called pleural chemotherapy, or, abdomen, called intraperitoneal chemotherapy. You may also get a second course of chemotherapy, called “second-line” chemotherapy, with pemetrexed or other drugs, raltitrexed plus oxaliplatin, or the triple drug combination of irinotecan, cisplatin and mitomycin.

Radiation

Radiation can be an important part of mesothelioma treatment. The problem is that because the cancer is near the heart and lungs, it’s challenging to provide the kind of high-dose, intensive therapy needed to shrink the tumor. However, a newer option, intensity-modulated radiotherapy (IMRT), which can more accurately target cancer cells and avoid healthy tissue, may provide better results when performed by experienced clinicians.

Investigational therapies are being explored through clinical and surgical trials at many of the nation’s top cancer centers.

There are more than 50 studies on new therapies for mesothelioma in the U.S. that are looking for volunteers. Researchers are investigating new targeted drugs and chemotherapies, as well as new protocols for giving the medications; immunotherapy, which harnesses the power of the immune system to fight the disease; phototherapy, in which you are injected with a drug that bonds to cancer cells and is activated by high-intensity light; genetic therapies; and novel radiotherapy techniques like tomotherapy to treat the disease. Sejarah Proklamasi Kemerdekaan Indonesia

Pada tanggal 6 Agustus 1945 sebuah bom atom dijatuhkan di atas kota Hiroshima di Jepang, oleh Amerika Serikat yang mulai menurunkan moral semangat tentara Jepang di seluruh dunia. Sehari kemudian BPUPKI berganti nama menjadi PPKI (Panitia Persiapan Kemerdekaan Indonesia) untuk lebih menegaskan keinginan dan tujuan mencapai kemerdekaan Indonesia. Pada tanggal 9 Agustus 1945, bom atom kedua dijatuhkan di atas Nagasaki sehingga menyebabkan Jepang menyerah kepada Amerika Serikat dan sekutunya. Momen ini pun dimanfaatkan oleh Indonesia untuk memproklamasikan kemerdekaannya.
Soekarno, Hatta selaku pimpinan PPKI dan Radjiman Wedyodiningrat sebagai mantan ketua BPUPKI diterbangkan ke Dalat, 250 km di sebelah timur laut Saigon, Vietnam untuk bertemu Marsekal Terauchi. Mereka dikabarkan bahwa pasukan Jepang sedang di ambang kekalahan dan akan memberikan kemerdekaan kepada Indonesia. Sementara itu di Indonesia, pada tanggal 10 Agustus 1945, Sutan Syahrir telah mendengar berita lewat radio bahwa Jepang telah menyerah kepada Sekutu. Para pejuang bawah tanah bersiap-siap memproklamasikan kemerdekaan RI, dan menolak bentuk kemerdekaan yang diberikan sebagai hadiah Jepang. Syahrir memberitahu penyair Chairil Anwar tentang dijatuhkannya bom atom di Nagasaki dan bahwa Jepang telah menerima ultimatum dari Sekutu untuk menyerah. Syahrir mengetahui hal itu melalui siaran radio luar negeri, yang ketika itu terlarang. Berita ini kemudian tersebar di lingkungan para pemuda terutama para pendukung Syahrir.
Pada tanggal 12 Agustus 1945, Jepang melalui Marsekal Terauchi di Dalat, Vietnam, mengatakan kepada Soekarno, Hatta dan Radjiman bahwa pemerintah Jepang akan segera memberikan kemerdekaan kepada Indonesia dan proklamasi kemerdekaan dapat dilaksanakan dalam beberapa hari, tergantung cara kerja PPKI.[1] Meskipun demikian Jepang menginginkan kemerdekaan Indonesia pada tanggal 24 Agustus.
Dua hari kemudian, saat Soekarno, Hatta dan Radjiman kembali ke tanah air dari Dalat, Syahrir mendesak agar Soekarno segera memproklamasikan kemerdekaan karena menganggap hasil pertemuan di Dalat sebagai tipu muslihat Jepang, karena Jepang setiap saat sudah harus menyerah kepada Sekutu dan demi menghindari perpecahan dalam kubu nasionalis, antara yang anti dan pro Jepang. Hatta menceritakan kepada Syahrir tentang hasil pertemuan di Dalat.
Sementara itu Syahrir menyiapkan pengikutnya yang bakal berdemonstrasi dan bahkan mungkin harus siap menghadapi bala tentara Jepang dalam hal mereka akan menggunakan kekerasan. Syahrir telah menyusun teks proklamasi dan telah dikirimkan ke seluruh Jawa untuk dicetak dan dibagi-bagikan.
Soekarno belum yakin bahwa Jepang memang telah menyerah, dan proklamasi kemerdekaan RI saat itu dapat menimbulkan pertumpahan darah yang besar, dan dapat berakibat sangat fatal jika para pejuang Indonesia belum siap. Soekarno mengingatkan Hatta bahwa Syahrir tidak berhak memproklamasikan kemerdekaan karena itu adalah hak Panitia Persiapan Kemerdekaan Indonesia (PPKI). Sementara itu Syahrir menganggap PPKI adalah badan buatan Jepang dan proklamasi kemerdekaan oleh PPKI hanya merupakan 'hadiah' dari Jepang.

Pada tanggal 14 Agustus 1945 Jepang menyerah kepada Sekutu. Tentara dan Angkatan Laut Jepang masih berkuasa di Indonesia karena Jepang telah berjanji akan mengembalikan kekuasaan di Indonesia ke tangan Belanda. Sutan Sjahrir, Wikana, Darwis, dan Chaerul Saleh mendengar kabar ini melalui radio BBC. Setelah mendengar desas-desus Jepang bakal bertekuk lutut, golongan muda mendesak golongan tua untuk segera memproklamasikan kemerdekaan Indonesia. Namun golongan tua tidak ingin terburu-buru. Mereka tidak menginginkan terjadinya pertumpahan darah pada saat proklamasi. Konsultasi pun dilakukan dalam bentuk rapat PPKI. Golongan muda tidak menyetujui rapat itu, mengingat PPKI adalah sebuah badan yang dibentuk oleh Jepang. Mereka menginginkan kemerdekaan atas usaha bangsa kita sendiri, bukan pemberian Jepang.
Soekarno dan Hatta mendatangi penguasa militer Jepang (Gunsei) untuk memperoleh konfirmasi di kantornya di Koningsplein (Medan Merdeka). Tapi kantor tersebut kosong.
Soekarno dan Hatta bersama Soebardjo kemudian ke kantor Bukanfu, Laksamana Maeda, di Jalan Imam Bonjol no. 1. Maeda menyambut kedatangan mereka dengan ucapan selamat atas keberhasilan mereka di Dalat. Sambil menjawab ia belum menerima konfirmasi serta masih menunggu instruksi dari Tokyo. Sepulang dari Maeda, Soekarno dan Hatta segera mempersiapkan pertemuan Panitia Persiapan Kemerdekaan Indonesia (PPKI) pada pukul 10 malam 16 Agustus keesokan harinya di kantor Jalan Pejambon No 2 guna membicarakan segala sesuatu yang berhubungan dengan UUD yang sehari sebelumnya telah disiapkan Hatta.
Sehari kemudian, gejolak tekanan yang menghendaki pengambilalihan kekuasaan oleh Indonesia makin memuncak dilancarkan para pengikut Syahrir. Rapat PPKI pada 16 Agustus pukul 10 pagi tidak dilaksanakan karena Soekarno dan Hatta tidak muncul. Peserta rapat tidak tahu telah terjadi peristiwa Rengasdengklok.

Peristiwa Rengasdengklok
Para pemuda pejuang, termasuk Chaerul Saleh, yang tergabung dalam gerakan bawah tanah kehilangan kesabaran, dan pada dini hari tanggal 16 Agustus 1945. Bersama Shodanco Singgih, salah seorang anggota PETA, dan pemuda lain, mereka menculik Soekarno (bersama Fatmawati dan Guntur yang baru berusia 9 bulan) dan Hatta, dan membawanya ke Rengasdengklok, yang kemudian terkenal sebagai peristiwa Rengasdengklok. Tujuannya adalah agar Ir. Soekarno dan Drs. Moh. Hatta tidak terpengaruh oleh Jepang. Di sini, mereka kembali meyakinkan Soekarno bahwa Jepang telah menyerah dan para pejuang telah siap untuk melawan Jepang, apa pun risikonya.
Di Jakarta, golongan muda, Wikana, dan golongan tua, yaitu Mr. Ahmad Soebardjo melakukan perundingan. Mr. Ahmad Soebardjo menyetujui untuk memproklamasikan kemerdekaan Indonesia di Jakarta. maka diutuslah Yusuf Kunto untuk mengantar Ahmad Soebardjo ke Rengasdengklok. Mereka menjemput Ir. Soekarno dan Drs. Moh. Hatta kembali ke Jakarta. Mr. Ahmad Soebardjo berhasil meyakinkan para pemuda untuk tidak terburu - buru memproklamasikan kemerdekaan. Setelah tiba di Jakarta, mereka langsung menuju ke rumah Laksamana Maeda di Jl. Imam Bonjol No. 1 (sekarang gedung perpustakaan Nasional-Depdiknas) yang diperkirakan aman dari Jepang. Sekitar 15 pemuda menuntut Soekarno segera memproklamasikan kemerdekaan melalui radio, disusul pengambilalihan kekuasaan. Mereka juga menolak rencana PPKI untuk memproklamasikan kemerdekaan pada 16 Agustus.

Pertemuan Soekarno/Hatta dengan Jenderal Yamamoto dan Laksamana Maeda
Malam harinya, Soekarno dan Hatta kembali ke Jakarta, bertemu dengan Letnan Jenderal Moichiro Yamamoto, komandan Angkatan Darat pemerintahan militer Jepang (Gunseikan) di Hindia Belanda dengan sepengetahuan Mayor Jenderal Otoshi Nishimura, Kepala Departemen Urusan Umum pemerintahan militer Jepang. Dari komunikasi antara Hatta dan tangan kanan komandan Jepang di Jawa ini, Soekarno dan Hatta menjadi yakin bahwa Jepang telah menyerah kepada Sekutu, dan tidak memiliki wewenang lagi untuk memberikan kemerdekaan.
Setelah itu mereka bermalam di kediaman Laksamana Maeda (kini Jalan Imam Bonjol No.1) untuk melakukan rapat untuk menyiapkan teks Proklamasi. Rapat dihadiri oleh Soekarno, M. Hatta, Achmad Soebardjo, Soekarni dan Sajuti Melik. Setelah konsep selesai disepakati, Sajuti menyalin dan mengetik naskah tersebut. Pada awalnya pembacaan proklamasi akan dilakukan di Lapangan Ikada, namun berhubung alasan keamanan dipindahkan ke kediaman Soekarno, Jalan Pegangsaan Timur 56[2](sekarang Jl. Proklamasi no. 1).
Sebelumnya para pemuda mengusulkan agar naskah proklamasi menyatakan semua aparat pemerintahan harus dikuasai oleh rakyat dari pihak asing yang masih menguasainya. Tetapi mayoritas anggota PPKI menolaknya dan disetujuilah naskah proklamasi seperti adanya hingga sekarang.
Para pemuda juga menuntut enam pemuda turut menandatangani proklamasi bersama Soekarno dan Hatta dan bukan para anggota PPKI. Para pemuda menganggap PPKI mewakili Jepang. Kompromi pun terwujud dengan membubuhkan anak kalimat ”atas nama Bangsa Indonesia” Soekarno-Hatta.

Detik-detik Pembacaan Naskah Proklamasi

Naskah asli proklamasi yang ditempatkan di Monumen Nasional dengan bingkai[3]
Perundingan antara golongan muda dan golongan tua dalam penyusunan teks Proklamasi Kemerdekaan Indonesia berlangsung pukul 02.00 - 04.00 dini hari. Teks proklamasi ditulis di ruang makan di kediaman Soekarno, Jl. Pegangsaan Timur 56 Jakarta. Para penyusun teks proklamasi itu adalah Ir. Soekarno, Drs. Moh. Hatta, dan Mr. Ahmad Soebarjo. Konsep teks proklamasi ditulis oleh Ir. Soekarno sendiri. Di ruang depan, hadir B.M Diah Sayuti Melik, Sukarni dan Soediro. Sukarni mengusulkan agar yang menandatangani teks proklamasi itu adalah Ir. Soekarno dan Drs. Moh. Hatta atas nama bangsa Indonesia. Teks Proklamasi Indonesia itu diketik oleh Sayuti melik. Pagi harinya, 17 Agustus 1945, di kediaman Soekarno, Jalan Pegangsaan Timur 56 telah hadir antara lain Soewirjo, Wilopo, Gafar Pringgodigdo, Tabrani dan Trimurti. Acara dimulai pada pukul 10:00 dengan pembacaan proklamasi oleh Soekarno dan disambung pidato singkat tanpa teks. Kemudian bendera Merah Putih, yang telah dijahit oleh bu Fatmawati, dikibarkan, disusul dengan sambutan oleh Soewirjo, wakil walikota Jakarta saat itu dan Moewardi, pimpinan Barisan Pelopor.
Pada awalnya Trimurti diminta untuk menaikkan bendera namun ia menolak dengan alasan pengerekan bendera sebaiknya dilakukan oleh seorang prajurit. Oleh sebab itu ditunjuklah Latief Hendraningrat, seorang prajurit PETA, dibantu oleh Soehoed untuk tugas tersebut. Seorang pemudi muncul dari belakang membawa nampan berisi bendera Merah Putih (Sang Saka Merah Putih), yang dijahit oleh Fatmawati beberapa hari sebelumnya. Setelah bendera berkibar, hadirin menyanyikan lagu Indonesia Raya.[4]. Sampai saat ini, bendera pusaka tersebut masih disimpan di Museum Tugu Monumen Nasional.
Setelah upacara selesai berlangsung, kurang lebih 100 orang anggota Barisan Pelopor yang dipimpin S.Brata datang terburu-buru karena mereka tidak mengetahui perubahan tempat mendadak dari Ikada ke Pegangsaan. Mereka menuntut Soekarno mengulang pembacaan Proklamasi, namun ditolak. Akhirnya Hatta memberikan amanat singkat kepada mereka.[5]

Pada tanggal 18 Agustus 1945, Panitia Persiapan Kemerdekaan Indonesia (PPKI) mengambil keputusan, mengesahkan dan menetapkan Undang-Undang Dasar (UUD) sebagai dasar negara Republik Indonesia, yang selanjutnya dikenal sebagai UUD 45. Dengan demikian terbentuklah Pemerintahan Negara Kesatuan Indonesia yang berbentuk Republik (NKRI) dengan kedaulatan di tangan rakyat yang dilakukan sepenuhnya oleh Majelis Permusyawaratan Rakyat (MPR) yang akan dibentuk kemudian.

Setelah itu Soekarno dan M.Hatta terpilih atas usul dari otto iskandardinata dan persetujuan dari PPKI sebagai presiden dan wakil presiden Republik Indonesia yang pertama. Presiden dan wakil presiden akan dibantu oleh sebuah Komite Nasional.
Isi Teks Proklamasi
Isi teks proklamasi kemerdekaan yang singkat ini adalah:
Kami bangsa Indonesia dengan ini menjatakan kemerdekaan Indonesia.
Hal-hal jang mengenai pemindahan kekoeasaan d.l.l., diselenggarakan
dengan tjara seksama dan dalam tempo jang sesingkat-singkatnja.

Di sini ditulis tahun 05 karena ini sesuai dengan tahun Jepang yang kala itu adalah tahun 2605.

Tapi perjuangan belum selesai seketika itu..., karena masih banyak coba'an2 yang di hadapi bangsa Indonesia. yang paling jelas dan hebat adalah ingin munculnya lagi belanda ke Indonesia. dan mereka di dukung oleh pasukan2 british dengan Ghurka nya mencoba mendarat di Sura baya. maka disitulah terjadi pertempuran hebat pada tanggal 10 november karena seorang perwira british yang terbunuh yaitu brgadir jendarl Mallaby. nah disitulah terlihat jelas betapa bangsa Indonesia benar2 suatu bangsa yang tidak mau terjual harga dirinya. walaupun mungkin Indonesia bisa maju jika diperintah oleh bangsa2 asing..., tapi kita memilih ingin berdikari apapun jadinya.., lebih baik mati daripada di jajah. lebih baik jadi harimau selamaya walaupun susah, ketimbang jadi keledai selamanya walaupun hidup senang. Itulah arti daripada suatu hakekat kemerdeka'an yang sejati...
